EXPENSE REPORT/CHECK REQUEST

This form is to request budgeted funds and reimbursement from the PTA.  See Budget tab for detailed line items.

TEACHERS/STAFF SECTION:  PTA has budgeted $6375 ($375 ea.) for 17 Teachers (K-5th)   and $1800 ($100 ea.) for other Teachers/Staff/Dept.'s. (See Budget posted in workroom.)  These funds REQUIRE Principal Signature.  (Turn this completed form into Principal and Principal will turn in to PTA.)

NOTE:  These funds can be used for EDUCATIONAL ITEMS, REWARDS, and/or INCENTIVES.  They can even be combined for larger purchases or a 2nd field trip, as the district provides the 1st field trip.

UNBUDGETED FUNDS REQUEST

(Accepted September-April) - This form is to request unbudgeted funds from the PTA.  (Funds in budgeted categories are paid through the Expense Report/Check Request.) Your request will be reviewed at the next General Meeting and will get a yes or no vote, unless further information or consideration is requested by the PTA.  Please allow ample time for this process.  Attach any research you have done on cost and include enough detail about how the funds will be spent to ensure the board/members has sufficient information to reach a decision.  PTA funds must be used to benefit Sacajawea Elementary students’ health, welfare, safety, or education and your request will be considered in light of those requirements.

Please return completed form to the PTA Drawer (school workroom) at least two weeks before the bi-monthly PTA General Meeting. 

Applicants are encouraged to attend the meeting and present their request in person.

CASH BOX FUNDS REQUEST 

Every PTA at some time will need cash for an event.  In PTA Money Matters it suggests a Cash Box form to help with this process.  The Treasurer will write a check to the individual (payee) not for cash, who is responsible for getting the cash for the cash box.  As with all checks, two signatures are required.  If it is the Treasurer who will get the cash, it is recommended that two different signers sign the check.  Since a receipt is always required, below is the form to fill out and use as a receipt.
